The Government Employees Medical Scheme (GEMS) was launched in 2005 to provide accessible, affordable medical aid to South Africa’s public service employees and their families.

As the second-largest medical aid scheme in the country, GEMS offers six plan options: Tanzanite One, Beryl, Ruby, Emerald, Emerald Value, and Onyx. Each plan is designed to accommodate different affordability levels while ensuring access to core healthcare services – including mental health and addiction treatment. GEMS is a restricted (closed) medical scheme, available exclusively to government employees.

GEMS has 822,901 members and 2,201,429 beneficiaries, according to the most recent financial performance industry report published by the Council for Medical Schemes (CMS).

Does GEMS cover rehabilitation?

Yes. GEMS covers inpatient treatment for addiction rehabilitation as part of its Prescribed Minimum Benefits (PMBs). This applies across all benefit options, as long as the treatment is deemed clinically appropriate and pre-authorisation is granted.

  • Up to 3 days of in-hospital medical detoxification
  • Up to 21 days of inpatient rehabilitation per calendar year
  • Residential psychiatric care for substance use disorders
  • Group therapy and individual counselling
  • Post-acute step-down services (within the 21-day limit)

Additional days may be considered on motivation if clinically justified, especially where dual-diagnosis (e.g., addiction and depression) is present.

Does GEMS cover psychiatric care?

Yes. GEMS covers inpatient psychiatric treatment as part of its Prescribed Minimum Benefits (PMBs), which apply across all plan options. This includes care for conditions such as depression, anxiety, bipolar disorder, trauma-related disorders, and dual-diagnosis cases involving substance use.

GEMS typically funds up to 21 days of inpatient psychiatric care per year, which may include a combination of detox, stabilisation, therapy, and medical management. Additional days may be authorised based on clinical need and the submission of progress reports.

At Houghton House, psychiatric care is provided by a multidisciplinary team that includes psychiatrists, psychologists, social workers, and nursing staff.

Houghton House is not currently a designated service provider (DSP) for GEMS. However, we are a registered provider and a member of the National Hospital Network (NHN), which means all services are billed at GEMS scheme rates.

This means GEMS members can receive inpatient addiction treatment at Houghton House with full benefit cover, provided the treatment is preauthorised. We work directly with GEMS to manage authorisation, treatment plans, and clinical reporting throughout your stay.

No referral is needed. You can contact Houghton House directly. We begin with a confidential assessment conducted by one of our registered mental health professionals. We submit the necessary documents and motivation to GEMS for pre-authorisation. Our team handles all paperwork and clinical coding, so there’s no administrative burden on you or your family.

No. All treatment at Houghton House is provided at GEMS scheme rates, and we waive any shortfalls or co-payments. The only cost to you may be a once-off admission fee and any copayments for medication. Everything else, including assessments, inpatient care, detox (if needed), and therapy, is included.

GEMS requires pre-authorisation before treatment to confirm that it is medically necessary and eligible for cover. At Houghton House, this is handled entirely by our admissions and case management team.

After a professional clinical assessment, we submit a motivation and diagnosis directly to GEMS along with a proposed treatment plan. Once approved, we continue to submit progress reports throughout your stay to ensure uninterrupted cover. You won’t need to manage any of this yourself; we take care of it for you.

Waiting periods may apply if you recently joined GEMS or rejoined after a gap in membership. These include:

  • A 3-month general waiting period for new members or dependents
  • A 12-month condition-specific waiting period for pre-existing conditions

However, PMB-related care (including treatment for substance use and psychiatric conditions) may still be covered during waiting periods. Houghton House will verify your benefit status and advise you upfront if any waiting periods affect your admission or cover.
